Principles of person centered care

Four Seasons Health Care is committed to delivering Person Centred Care and as such all staff are trained in this philosophy. By adopting a person centred approach to care the services we offer is far more flexible and responsive. This is achieved by ensuring that all care plans fully take into account the residents expressed preferences.

As a result every resident receives care specifically designed to meet his or her needs with the emphasis on enhancing the well-being of the individual and whilst not on trying to force the recovery of skills which may have irrevocably gone promoting independence wherever possible.

Prior to admission, each new resident is encouraged to visit the home to satisfy themselves that the ambience is to their liking.

Our Home Manager will also wish to discuss the personal preferences of each prospective resident and whatever care and other needs they may have. This is essential to confirm that we are able to provide the correct environment for each individual.

Every resident in our Care Homes is assigned a key worker who works with the resident, the named nurse (in nursing establishments) and the family where appropriate to ascertain information which will help to provide appropriate care. As result care plans in our care homes are totally person centred i.e. they will state what the person wants or needs and what they don’t want or need based upon their unique life history, personality and preferences expressed before the onset of whatever condition has lead to them needing the required level of care.

Care plans are then reviewed regularly reviewed to make sure needs continue to be met. Residents, and where appropriate their families, are involved as far as is practical in the decisions regarding their care.

Person centred care also means that as far as possible we are able to encourage residents to decide how they wish to live during their stay with us.

For example ‘You choose when you want to get up and go to bed, which activities you wish to join in with and what and where you prefer to eat’.
