We are committed to helping build links between the public and independent sector and we continue to seek ways to develop intermediate
care services and improve the opportunities for rehabilitation of service users.
In our view, intermediate care is an intensive short-term service that is about promoting independence, so that people avoid admission to hospital, or can return to their own home after rehabilitation.
The programmes principle of helping people to recover in pleasant surroundings, with appropriate care according to individual needs, prior to resuming independent living, has already been proven to work in practice.
We are completely flexible in talking and working to provide a service that is tailored to providing solutions to your requirements.
The average length of stay for patients with us so far has been three to eight weeks and over 95% of patients admitted from hospital for post operative rehabilitation have returned to independent living.

Enabling and Sub Acute Medical Facilities
In conjunction with a partnership of specialist service providers we have three different models of intermediate care that can be tailored according to the commissioner's requirements.
All models can stand alone but are best deployed as part of a strategic hospital admission avoidance programme, co-ordinating all community and home service providers to create a "virtual community hospital" that systematically assesses patients' needs and requirements leading to a more cost effective use of the acute hospitals' resources.
Plus there is the opportunity for joint profit sharing ventures with PCT or PBC clusters if preferred.
Admission to a facility can be planned or an acute admission. Prior to admission an assessment
is carried out either in the individual's own home, hospital ward or elsewhere.
Information will be gathered also from family carers, general practitioners, hospital
consultants, social services, care managers, or community nurses. This will result
in establishing an agreed partnership programme of care.
