You'll find Lawn Park in an idyllic position, overlooking the park in Sutton-in-Ashfield near Mansfield, Nottinghamshire. Here, our team of registered nurses and well trained, dedicated care assistants provides excellent quality 24-hour care.

We serve different levels of care, including nursing, residential care, rehabilitation care, post-operative care, and palliative care.

It's just a short walk to a bus route into the main town. We have pleasant, easy-to-access gardens visited by wildlife, and a large parking area for visitors to the home.

We have two floors furnished to an excellent standard. There's a large and comfortable lounge, conservatory, quiet lounge and a dining room with garden views. All our bedrooms have en-suites.

As delayed transfers of care remain at record highs, how can we reduce unnecessary hospital stays for older people? That was the key question at an HSJ event held in association with Four Seasons Health Care.

The roundtable heard that the healthcare system often finds reasons not to deal with care home residents and looked at the success of an innovative collaboration between the NHS and care homes in Belfast.

With a collaborative approach between care homes and the NHS, older people are able to leave hospital and receive expert care, whilst assessing whether their long term future would be in their current home or a local care home. “The idea is that it would provide short term rehabilitation in conjunction with the two trusts in that area. The joint partnerships with the trusts I’ve worked with have just been amazing, and that’s what makes it work”, explained Janice Brown, Four Seasons’ regional manager for the Belfast and Newtownabbey area.

In addition to the improvements from a collaborative solution, South Eastern Health and Social Care Trust reported that the joint partnership approach of providing a bed in a care home reduced their yearly cost by more than half.

Lawn Park Care Home in Sutton in Ashfield is celebrating the success of Care Assistant Jamie Rogerson after he successfully graduated from the Care Home Assistant Practitioner (CHAP) programme.

Rosalind Kennedy-Brown, Home Manager said: “Everyone here congratulates Jamie on his success. We’ve supported him every step of the way and it’s great to see his hard work pay off. He is a great asset to the home, and provides fantastic support to the residents here. He is a great member of the team. Well done again! I can’t wait to see you putting your new skills to use.”

The newly qualified CHAPs are now able to provide some general nurse duties, such as medication rounds. This benefit’s the home as it enables the nurse to deal with more complex issues.

You have come through this long journey, and I want to thank you for your commitment and continued dedication. I also was to thank the mentors for their continuous help and support.

People embark on a career path because it’s something they love, they yearn to develop, grow and blossom in their profession.

Here at Four Seasons Health Care, Care Assistants can up-skill themselves on the organisations Care Home Assistant Practitioner (CHAP) Programme.

On Thursday 2nd March 2017, a graduation ceremony was held at in Doncaster, celebrating the achievements of 29 newly qualified CHAPs.

Sam Jacob, Managing Director for Four Seasons in the East of England, said: “Here at Four Seasons Health Care we take great pride in our CHAP Programme, and everyone who undertakes the intense 12 week course. I want to congratulate all 29 of the CHAPs for graduating; your hard work and dedication have paid off. These are the first CHAPs in my area, and I’m absolutely delighted at all of your achievements.

“The Programme is innovative and helps the CHAPs to provide invaluable support to the nurses in the homes.

“All the CHAPs have come through this long journey and I want to thank you for your commitment. I also want to thank the Nurse mentors who have guided and supported the CHAPs every step of the way. Well done everyone!”
