Towards the end of January many people’s New Year’s resolutions begin to wane. Not the determination of the residents at Flower Park Care Home in Doncaster.

“The residents have started taking part in a weekly chair based exercise class here at the home,” said Home Manager Amanda Searle. “The residents really enjoyed themselves. They were having fun whilst keeping themselves supple. Who said living in a care home was boring?”

Since 2015, Harrogate Lodge Care Home in Leeds has been working with Leeds Clinical Commissioning Group to provide an intermediate care service for older people in the local community.

The service provided at Harrogate Lodge empowers the individuals it supports by working with them to set their own rehab goals, establishing appropriate discharge dates, addressing their psychological and emotional well-being whilst meeting their health and care needs.

What sets Harrogate Lodge aside is the fact that it is supported by a dedicated GP, a consultant geriatrician, its own occupational and physiotherapists and support from community health teams. These professionals meet up three times a week with the individual to ensure all aspects of their needs are being met. This constant review ensures that individuals who use their service are able to meet their discharge dates.

This service has gone from strength to strength. From November 2015 to July 2016, the service has supported 108 individuals with an average stay of 25 days. Between April 2016 to July 2016 the service supported 42 individuals with an average length of stay of 11 days.

More information about the intermediate care service that we provide can be found on our website.
