Springfield Care Home in Blackburn is working with Darwen Clinical Commissioning Group in the provision of a 10 bedded sub-acute rehabilitation unit. The unit provides rehabilitation, usually following an acute episode of care for either emergency or elective treatment.
The reason for providing intermediate care in the Blackburn area is to support individuals who have experienced an acute illness or trauma and following hospital treatment have been identified for further therapy that will enable a safe return to their own home. Patients who use this service will have been identified as needing additional therapy to help them achieve the best results, away from a hospital ward.
The unit at Springfield is very busy, with an average stay of 14 days for a patient with over 60% of them returning to their own homes and living independently.
