Welcome to Swan House Care Home

Swan House, in the historic market town of Chatteris, Cambridgeshire, offers a range of levels of care, from residential to general nursing. Our team of nurses and well-trained senior carers and care assistants offers 24-hour care to a very high standard.

Our purpose-built home has 16 residential beds, one intermediate care bed overseen by local GPs, and 22 nursing beds. There are spacious lounges and dining rooms for residents to enjoy. We also have a dedicated activity room and a hairdressing salon.

All bedrooms have en-suites, telephone links and great safety systems. We love residents to personalise their rooms to make them feel more homely.

We have a secure, walled, traditional garden for everyone to use, and also a Wii system which is proving to be quite a hit.

Four Seasons Health Care picked up three accolades on Tuesday 5th December 2017 at the Training Journal (TJ) Awards. Winning three categories meant that Four Seasons Health Care led the way, as they were the only organisation to win in multiple categories.

Who were the winners?

The Dementia Care Framework won the ‘Best Change Management Programme’, which is a fantastic achievement and further highlights the positive impact this is having on the lives of people living in Four Seasons homes with dementia.

The Judges commented: “We recognise Four Health Care work in the field of dementia care as innovative and thorough in its approach, being both effective and transformative. It had a bold vision that was well executed in a challenging environment.”

The Care Home Nursing Journal (CNJ), Four Seasons’ very own internal publication to develop nurse knowledge and share best practice won its category of ‘Best Organisational Development Programme'.

The Judges commented: “The design of this intervention, which on the surface appears very simple, completely meets the needs of the organisation and has had significant impact. A particular strength was that Four Seasons Health Care decided to make these resources freely available to anyone, making the impact even greater and is improving healthcare outcomes in care homes everywhere.”

Finally, Louise Morris the Care Home Assistant Practitioner (CHAP) Lead won the main award of the night and was crowned the ‘Learning and Development Professional of the Year’.

The Judges stated: “Louise has had a transformational impact, not only on the learning and development of staff at Four Seasons Health Care, but also on the well-being of residents. Through a review of an existing programme, Louise drove design and improvement which has had significant benefits through improved care, staff practice and confidence and commitment to their work and professional development.”

Residents and the care team at Swan House Care Home in Chatteris took a step back in time as they travelled down memory lane to the 1940s.

Everyone in the home went all out and embraced the theme, with the care team dressing up in fancy dress, a singer who belted out hits from the era and 1940s themed food.

Rachel Bates, Home Manager, said: “Everyone had a great time. We looked over some war memorabilia, and we looked at air raid sirens, Woodbine cigarettes and many other items which helped our residents to reminisce about their younger years. When our fantastic entertainer left we then put on a war time move which went down a real treat with the residents.”

There’s nothing more relaxing than partaking in a guilty pleasure. Food, music, films, reading etc. Everyone has a guilty pleasure.

Two residents at Swan House Care Home in Chatteris enjoy a little bit of rock and roll, and indulged in this guilty pleasure recently at the home.

Rachel Bates, Home Manager, said: “We strive to provide special experiences for our residents on a regular basis. Norma and Doug both enjoy rock and roll, so they had an opportunity to rock out in the home complete with inflatable instruments. The two residents thoroughly enjoyed themselves, and said it made a nice change to the ‘boring’ music that can sometimes be heard in the home.”
