Welcome to Ty Gwynno Care Home

Ty Gwynno is a modern, purpose-built home in Hopkinsontown, near Pontypridd, Mid-Glamorgan, providing high-quality care, 24 hours a day. We pride ourselves on creating a friendly, informal atmosphere where residents and guests can relax and feel at home.

We look after residents with varying needs, from specialist nursing support to general assistance with everyday living. Some people stay with us long term; others join us for shorter periods of time, often to give their carers much-needed respite. Our nursing team offers residential and nursing support, as well as specialist care for those living with dementia.

We encourage residents to bring items from their own homes, including any pets as these are part of the family too. It's very important to us that each and every resident makes Ty Gwynno his or her new home.

Residents happily pursue their hobbies in various communal and quiet lounges dotted throughout the home. They also love the stunning views over the Welsh valleys and mountains from the patio area of our delightful landscaped gardens.

There are many areas where residents can relax, socialise and invite friends and family to enjoy when they visit. Our bright, open-plan reception area is a great place to meet before heading out for the day.

Ty Gwynno has an excellent reputation in the local community, which is very supportive and sympathetic to the needs of residents. We have built up strong relationships with social groups and places of interest, to make sure people get the very best out of living with us.

Life at Ty Gwynno Care Home

A carer and a resident

I chose Ty Gwynno for my Aunt as I felt the atmosphere and professionalism of the staff and manager were second to none. As soon as I entered the home I was made to feel very welcome. I hadn't made an appointment to visit - just turned up and this wasn't frowned upon like it had been at a nearby home.
I was given a tour of the home and was extremely impressed with the warmth and friendliness of all the staff. The care being given to the residents was exemplary - it seemed that nothing was too much trouble. I was even offered a cup of tea and a piece of cake!
The manager was very knowledgeable and showed great understanding of my Aunt's condition and completely put my mind at ease. It is a very difficult decision to make (having to admit that your nearest and dearest need to enter a Care Home) but Sandra made me realise that I was making the correct decision for my aunt and that she would be very well looked after at Ty Gwynno.
I cannot praise this home highly enough and thoroughly recommend it. It really is a "home from home".

P Frost, Nephew of Resident/Service User at Ty Gwynno Care Home

Our elderly mother, who has Alzheimer's, spent several weeks at Ty Gwynno until 17th October 2014. She was made to feel welcome and very much at home there. The staff are not only well trained and good at their jobs, but also very caring and concerned about their residents. It is an excellent care home and my brother and I would not hesitate to recommend it to others.

Four Seasons Health Care picked up three accolades on Tuesday 5th December 2017 at the Training Journal (TJ) Awards. Winning three categories meant that Four Seasons Health Care led the way, as they were the only organisation to win in multiple categories.

Who were the winners?

The Dementia Care Framework won the ‘Best Change Management Programme’, which is a fantastic achievement and further highlights the positive impact this is having on the lives of people living in Four Seasons homes with dementia.

The Judges commented: “We recognise Four Health Care work in the field of dementia care as innovative and thorough in its approach, being both effective and transformative. It had a bold vision that was well executed in a challenging environment.”

The Care Home Nursing Journal (CNJ), Four Seasons’ very own internal publication to develop nurse knowledge and share best practice won its category of ‘Best Organisational Development Programme'.

The Judges commented: “The design of this intervention, which on the surface appears very simple, completely meets the needs of the organisation and has had significant impact. A particular strength was that Four Seasons Health Care decided to make these resources freely available to anyone, making the impact even greater and is improving healthcare outcomes in care homes everywhere.”

The CNJ is available to view on the Four Seasons website here.

Finally, Louise Morris the Care Home Assistant Practitioner (CHAP) Lead won the main award of the night and was crowned the ‘Learning and Development Professional of the Year’.

The Judges stated: “Louise has had a transformational impact, not only on the learning and development of staff at Four Seasons Health Care, but also on the well-being of residents. Through a review of an existing programme, Louise drove design and improvement which has had significant benefits through improved care, staff practice and confidence and commitment to their work and professional development.”

More information about the role of a CHAP is available on the Four Seasons website here.

Congratulations to everyone on this fantastic achievement!
